When creating your custom forms, you have access to various cell controls. Some controls offer the ability to configure logic flow. Logic control and flow for example, might be a selection of a radio button in one cell that disables the function of another. Where ‘Logic’ is available, it is listed as ‘L’ or under the section entitled ‘Conditional Logic’. Logic control is not available on 'Checkbox List'.
All forms are output with standard font and cell colours as in the following example
When creating forms and following the selection of a form row/cell, you will be presented with the following list of controls:
Provides a large font size for title areas of your form, as in this example of ‘Control of Asbestos’. There are no formatting options; the size and colour are static.
Heading Edit view
Heading Form view
Used for narrative or instruction about form elements. Formatting controls comprise Bold, Underline, Italic, Subscript and Superscript, along with bullet points and numbered lists.
Paragraph Edit view
Paragraph Form view
Used for free form text entry when the form is in use. This for example would be used to request input for details about an issue.

Textbox options:
Show Title – adds a title to the entry field
Required – Form validation. The form will not be accepted as complete until detail has been entered in to this field. In form view, this field will be listed with a red asterisk.
Multiline / Height (Lines) – By default, a text box field is one character high
Live Data – Field Link, used in portal forms to dynamically update patient records from data entered - This will overwrite existing data in the patient record.
Conditional Logic – used to control form flow
Allows for numeric data entry option with range control.
Numeric / Numerical Range Edit view

Enables choice of selection by a drop down list. Only one answer selection is permitted.

Dropdown Form view
Similar in function to a dropdown list but all answer options are available at once. Enables choice of selection by radio button. Only one answer selection permitted.

Radio List options:
Show Title – adds a title to the entry field.
As Horizontal – Unchecked displays buttons on vertical alignment. Checked, displays horizontal.
Required – Form validation. The form will not be accepted as complete until detail has been entered in to this field. In form view, this field will be listed with a red asterisk.
Live Data – Field Link, used in portal forms to dynamically update patient records from data entered - This will overwrite existing data in the patient record.

Enables choice of selection by check box. Multiple choice permitted.

Provides a date picker for a single date selection or as ‘from’ and ‘to’.

Allows the upload of an image and selection of whether drawing is permitted. Drawing here for example might be used to highlight a problem area on an anatomical diagram.
Image Edit view
Image options:
Show Title – adds a title to the entry field
Image File: - Provides file explorer for image selection and upload
Allow Drawing: - Enables selection of colour and basic drawing to mark up/over image

This option is only available with Client Portal forms (Contact and Patient). It provides Form Builder the ability to upload additional external material to the form as it’s being completed, for example, a sick note when completing an absence type form.

‘Choose file’ presents a file explorer in order to select the relevant file for upload.
