Form Builder – Portal Forms

Creating Portal Forms

In the ‘Basics’ examples you would have seen how to build ‘Module’ forms for use in your consultations/appointments.  Here we will focus on forms for the Portal.  

Start by selecting the ‘Add a new assessment button’ from the top left of the page.

By selecting the ‘Type’ option as ‘Portal Form’, you will also have the option to select whether it should be a Contact or Patient form.  A ‘Contact’ in this instance might be a referring manager of a client to whom you provide services.  A ‘Patient Form’ relates to forms such as pre placement forms; those that would be completed by the end subject, an employee/patient for example.  

The tick box listed as ‘Create Case’ engages with the case management feature and can be used to track returned forms as a type of ‘triage’ for filtering.  For example, when numerous management referral type forms are received each will be listed in tabular form and can be marked as accepted or rejected (due to incomplete information), filtered, reported and acted on accordingly.

The selection of ‘Portal Form’ will present available clients in the right column as ‘Associated Clients’. This column allows you to quickly select the client to which the new form should be associated, if any.  Please review the ‘Portal’ articles for more information on configuring clients for access and form availability on the portal.

You can continue to build forms in exactly the way as in the ‘Basics’ examples.
